I hereby consent to members of my family participating in the Lil' Sports programs. I recognize that there is an assumption of risk when participating in sports, and understand that injuries, including permanent paralysis or death can happen when participating in activities involving height and/or motion, including but not limited to stretching, karate, sports, dance, gymnastics and tumbling and trampoline activities. That said, I agree to make myself/child(ren) aware of the possibility of injury and encourage my child(ren) to follow all the safety rules and the Lilâ€™ Sports instructorâ€™s instructions, including but not limited to instructions posted and as verbally instructed by Lilâ€™ Sportsâ€™ staff members. I fully understand that Lilâ€™ Sports Franchising Group, Inc., its franchisees, affiliates and staff members are not physicians or medical practitioners of any kind. With the above in mind, I hereby allow Lilâ€™ Sports Franchising Group, Inc., its franchisees, and affiliate program staff to render first aid to me, or my child(ren) in the event of any injury or illness, and if deemed necessary by said staff to call for and to seek medical help, including transportation by ambulance at your expense if in the discretion of a Lilâ€™ Sports Franchising Group, Inc. franchisee, affiliate staff member or its representatives, it is deemed that such services are required. I understand that it is the expressed intent of Lilâ€™ Sports Franchising Group, Inc., its franchisees, and/or affiliates to provide for the safety and protection of my child(ren) and in consideration for allowing my child(ren) to participate in programs, I hereby release Lilâ€™ Sports Franchising Group, Inc., its franchisees, and/or affiliates, its officers, employees, and subcontractors, from all liability for any and all damages and injuries suffered by my child(ren) while under the instruction, supervision or control of Lilâ€™ Sports Franchising Group, Inc., its franchisees and/or affiliates. I also affirm that I now have and will continue to provide proper hospitalization, health and accident insurance coverage which I consider adequate for my child(ren) and my own protection. I understand that it is the parentsâ€™ responsibility to warn the child about the dangers of gymnastics and potential injury. I also understand that safe, professional instruction often includes hands-on spotting to my child. This acknowledgement of risk and waiver of liability, have and been read thoroughly and understood completely and signed voluntarily as to its content and intent. I understand there are no cash refunds, and agree to follow any policies regarding participation, as stated by Lilâ€™ Sports Franchising Group, Inc., its franchisees, and/or affiliates. I have had the opportunity to ask questions regarding this Acknowledgement of Risk and Waiver of Liability/Assumption of Risk and acknowledge my right to have same reviewed by an attorney of my choosing.

(Optional) Permission to use child's photos in promotions.
I permit Lilâ€™ Sports Franchising Group, Inc., its franchisees and/or affiliates to use pictures of my child for its advertising, direct marketing, and web promotions, and understand they will NOT use my child(ren)â€™s full name in the aforementioned projects. Lilâ€™ Sports owns the rights to any and all images and no compensation will be paid to any party for images used in advertising, direct marketing and/or web promotions.
